Insurance Card

If your child is continuing CHIP enrollment, he or she will not receive a new insurance card.

If you lose your card, please call Blue Cross Blue Shield at (800) 447-7828 ext. 8647 to have it replaced.

If your child is a new CHIP enrollee:

  • You will receive an insurance ID card, a member handbook and a provider directory from Blue Cross Blue Shield between the 10th and 14th of the month enrollment begins. For example, if you receive a letter from CHIP dated January 10 stating your children will be enrolled February 1, you can expect the packet around February 10.

  • If your child has a medical appointment after enrollment begins but prior to receiving an ID card, take your letter with you to the medical provider.

  • If your child needs a prescription filled prior to receiving the ID card, you may contact Blue Cross Blue Shield at (800) 447-7828 extension 8647.

  • As proof of CHIP coverage, show your dentist your CHIP ID card. Your dentist will bill CHIP according to the instructions in the CHIP Dental Provider Manual.

How do I find a medical provider in my area?

  • To search for a medical provider in your area, click on www.bcbsmt.com/index.htm

    Click on "Find a Doctor" and select "CHIP" as the plan name.

Dental Providers In My Area

How do I find a CHIP dentist in my area?

  • You can get the names and phone numbers of CHIP dentists by calling the CHIP Family Health Line at (877) 543-7669 or (406) 444-6971.

  • You can view a list sorted by city. You will need Adobe Acrobat Reader to view this list.

  • If your current dentist is not on our list, you can ask if he or she would be willing to become a CHIP dentist. If yes or if more information is needed, the dentist can call either of these numbers and we'll help get the paperwork started! (800) 624-3958 (ACS Provider Relations) or (406) 444-5288.

Vision and Eyeglasses

How do I find a vision care and/or eyeglass provider in my area?

  • Vision and eyeglass providers are listed in your CHIP provider directory or you can click on the following link www.bcbsmt.com/index.htm Click on "Find a Doctor," select ophthalmology, optician, or optometry as a specialty, and select "CHIP" as the plan name.

CHIP Extended Dental Plan

The Extended Dental Benefits Program (EDP) offered under the Children’s Health Insurance Plan (CHIP) was extremely successful.  All funds for this benefit year (Oct 1-Sept 30) are now committed and 671 CHIP children with significant dental needs have or will receive additional dental care beyond the CHIP basic dental benefit.  CHIP is no longer accepting any new “Requests for Extended Dental Benefits” since all funds are committed.   

CHIP’s basic dental plan remains in effect for each CHIP covered child.  That plan is limited to $412 in billed charges per benefit year (Oct 1 – Sept 30).  Providers should verify a child’s CHIP eligibility prior to providing services.

CHIP will notify providers and CHIP families when CHIP Extended Dental Program funds are available again.

  • Add or remove family members.

  • Change in insurance status.

  • Change of address.
